Lathe Screw Threading Issue.
I think john.k is closest to the problem here. From reading the thread charts and the dial chart... that is clearly a 4TPI imperial lead screw. The thread dial chart shows "16 1-8 " which means that any position is allowed on the thread dial. This is the same for TPIs that are whole multiples...- David Utidjian

Is your shaft straight, true, and concentric to begin with? Looks like that one has traveled over a shop floor once or twice and been traveled over itself by some heavy blunt objects. What is this and what is it used for
My first impression is that it is a nice wooden box. Usually one puts stuff in boxes. Fortunately that one has the FSN# which if you google it you will find it is a balancing kit for the main rotor of some sort of helicopter. Any more details I will leave up to you to discover.- David Utidjian

support points for flatness inspection on surface plate
I use machinist jacks with differential threads so I effectively have 16 TPI, 14 TPI, and 112 TPI adjustment from each jack. I recently needed to level my small surface plate in order to check my levels... sounds weird but you all know what I mean :D I have the usual Starrett and other brands...- David Utidjian

OK so the slot is 1/16 of an inch wide by 1/8 of a foot deep (or 1.5 inches). Yeah that is going to be a tough one. More seriously I am sure you mean 1/8 of an inch deep. Without being able to see the internal structure of that vessel wall I would say it is either a reaction vessel or holding vessel from a petrochemical plant. A design decision regarding the orientation of three bearings about a 3/4" bar.
