    Richard, I would use enough anchors to pull the 4x4s snug to the wall and mount the entire 4x4 to the wall with epoxy. Wire brush the mating part of the wall real well beforehand. It will never pull off unless the concrete comes with it. The epoxy that mixes in the nozzle would be real convenient.

    As above, Rotometals can supply what you need. Its likely not as expensive as epoxy products. Biggest trick might be whether you have a way to melt and pour it. About what volume do you need?

    The bar does not only bend when loaded it also twists. I think chatter is more a result of the twist being loaded and unloaded during the cut. Look real hard at the insert maker’s information for doc, speed and feed on your material and application.

    Well compacted crushed stone and a turn down edge. I would not make the slab bigger than the building. Unless you like leaks, you want the exterior finish overhanging the slab. I’d also contact the builder to let them know you might be late.
