Need information Supermax LE-1338 lathe

This pic is from the web. Can't blow it up and get usable data.

During transport the data plate for the lathe was lost.
It's a Supermax LE-1338 see attached pic. With the AC RT and XYZ black levers.
If anyone here has this lathe can you provide me with 2 high quality perpendicular and square pictures of the serial and thread pitch plate. One of the top and one op the bottom with a little overlap.
I want to get a new plate laser etched.
