Last thread pass ruining threads

The first DOC can be increased. There is no point in having the same DOC all along.
If the last few pass are found to be tough for the insert, possibly DOC can be an issue.

Maybe the first cut is too heavy.
His First DOC is only 0.001", as is his Minimum DOC. Therefore, the whole thread, except for whatever is left for the final pass, is going to cut with 0.001". Rule of Thumb, the First DOC should be whatever the Threading Tool and the Workpiece setup will constantly tolerate. 316 work hardens if the cutting tool is rubbing and unless new inserts are swapped in on a very regular basis, the tool has the potential to rub fairly early when using 0.001" DOC for the whole threading journey.

Added r word, which gave me what I wanted,
What many new chums don't understand, is that the arguments used in the first G76 Block set values in parameters. These values remain the same until another value is specified in the first G76 Block. By omitting any argument from that Block, the "R" argument in your case, their values set in previous G76 Cycles will be used. The Finish Allowance being used by your original program would have been whatever the "R" was when it was last specified in a previous G76 Threading Cycle.
